Evidence-Based Comprehensive Psychological, Nutritional and Medical Care

Discharge/Aftercare

When our patients are ready to discharge from our treatment programs, Laura Fewell, our discharge coordinator, will work with each individual patient to ensure a smooth transition to an outpatient treatment team.  When returning to your outpatient treatment team, we will help facilitate the process and maintain open communication with your team.  For patients that do not have an outpatient team upon admission, our discharge coordinators will work with you to assemble an outpatient team that meets all of your needs.

At McCallum Place we will keep in touch with you to ease the transition out of treatment by checking in with you for the first six months following discharge.  We also provide several free support groups that we encourage you to attend.
